Navigating the Fellowship Application Process: Multi-Institutional Experiences About How to Successfully Mentor and Support Residents

Each year, over 14,000 applicants pursue internal medicine (IM) subspecialty fellowship training via the NRMP Match. Supporting residents through this process requires a thoughtful, adaptable, and sustainable advising framework. In this workshop, we present a stepwise model developed across two academic institutions, each advising ~30 applicants annually across subspecialties. We’ll outline our timeline and advising approach, from early exploration of subspecialty interest and scholarly alignment to mentor identification, networking strategies, and guidance on application components. Our model includes fellowship workshops with subspecialists, alumni databases, and gap year normalization. Central to our approach is delivering individualized, honest feedback with empathy, helping residents gain clarity, resilience, and alignment with their professional goals. We leverage faculty expertise and real-time data, including NRMP trends and our own institutional post-match surveys, to provide personalized advising. We'll share our institutions’ survey insights and upcoming plans for improvements, including strategies for guiding residents on program signaling. Participants will receive PDFs of our resources and a one-page “Fellowship Application Mentorship Blueprint” including best practices and a recommended timeline, tools to help build or strengthen advising frameworks at their own institutions.
