Beginning July 2021, internal medicine began implementation of the revised Milestones 2.0. New to the revised milestones is PROF-4 "Knowledge of Systemic and Individual Factors of Well-Being." PROF-4 is meant to assess the extent to which trainees have insight into the institutional and personal factors that impact well-being. Given the prevalence of burnout and depressive symptoms in residency training across specialties, the development of a subcompetency centered on well-being represents an important addition to the toolbox for trainee assessment. However, assessing competence in PROF-4 poses a special challenge for program leadership. Flourishing-focused advising (FFA) is a technique to assess and develop competence in trainee wellbeing in the revised milestones [2]. In FFA, programs can build on the existing infrastructure of semiannual advisor and PD meetings to incorporate structured reflection on individual trainee well-being. During this process, the trainee completes a self-assessment of well-being and is prompted to consider the results through a reflective exercise before meeting with their advisor. This self-assessment is then used as a springboard for structured, intentional dialogue between the advisor and trainee during a semiannual review. In this workshop, we will introduce attendees to the FFA framework, explore methods of engaging trainees in conversations about the systemic factors affecting their well-being, and introduce a structured technique to assess and develop competence in trainee wellbeing.
